AGENDA ITEM DESCRIPTION
A Request for Proposals was announced in December 2014 accepting applications with a deadline of submission no later than January 29, 2015 at 5:00 p.m. A pre-submittal meeting was held with interested non-profit agencies at City Hall on January 7, 2015.

Goal of Non-Profit Funding Program:
The goal of the program is to create Partnerships with Community Organizations. This will be accomplished by providing health, human, and/or quality of life services to the residents of Maricopa. The non-profit funding requests recommended during each grant process fulfills the intent of the Maricopa Strategic Plan and designated City Council priorities while also leveraging City dollars.

Program Requirements:
· An agency must be able to provide Internal Revenue Service (IRS) letter sanctioning non-profit status.
· An agency must be able to provide liability insurance $1,000,000/$3,000,000/$1,000,000.
· Application requests may not exceed $25,000.
· Application may not exceed seven (7) pages and should be submitted typed in twelve (12) point font and one (1) inch margins.
· Applicants are required to attend a grant writing workshop and provide a certificate of attendance.
· Applicant must provide a completed City of Maricopa W-9 form with the application.
· Applicant must provide a signed offer page and any amendment documents with the application.
· Applicants must provide direct services which improve the health and welfare of Maricopa residents that have measurable outcomes.
